Pediatric Resource Center Connection

We are proud to share this from our friend Alyssa and her coworkers at the Milestone Pediatric Complex Care Resource Center NW about their new project. I work at Milestone Pediatric Complex Care Resource Center NW, in Warren, PA (same organization and building as the NW HCQU). The PCCRC project is a new statewide project. State info here--Pediatric Complex Care Resource Centers | Department of Human Services | Commonwealth of Pennsylvania

In a nutshell, our Family Facilitators work with families whose children are living in facilities, to try to put the supports in place so the children can move home (or move into a family home with a Lifesharing family, for instance). Our Pediatric Coaches work with those families who have those children already living at home, to provide support and training to help ensure the children can remain at home. PCs will also run Family Advisory Workgroups, which will be held a few times a year and also have state stakeholders in attendance (this project is brand new and these meetings haven't been held yet, so many details are still being worked out).

The target audience, in general, is children with medically complex conditions, ages birth-21, who are on Medical Assistance/Medicaid, and who qualify for at-home shift care nursing (though they may not be currently receiving such care due to a variety of factors). Typically, this would mean these children have conditions that affect several organ systems and require at least one piece of technology such as a trach, GI tube, or vent. Our services are provided at no-cost to qualifying families.

This is a statewide project; my location serves counties in NW PA, but anyone is welcome to reach out to any of the PCCRCs and we will connect them to the right location if necessary.

Family Facilitators

The Family Facilitator helps children with medical complexities from hospitals or facilities get back into the community and help to prevent institutional care by:

Meet with children, families, and providers in their homes and communities to assess needs and assist with the transition planning process.

Serve as a transitional coordinator until the child finds permanent placement in a family or community setting.

Connect with families, health organizations, hospital social workers, and discharge planners to discuss in-home care services and family-based alternatives to institutional care.

Collaborate with Health Care Systems, Managed Care Organizations (MCOS), and Department of Human Services (DHS) offices to accept referrals for children needing permanent family or community-based care.

Family Advisory Workgroups

The Pediatric Coach will also lead the Family Advisory Workgroups, designed to support families caring for children with complex medical needs. These workgroups offer families the chance to share valuable insights and feedback on the services they use including those provide by the PCCRC and other organizations. The Pediatric Coach will organize and host these meetings on a monthly or quarterly basis, offering options for virtual, in-person, or hybrid participation.
